Abstract |
The paper reports a case of a positive response to fibrinolytic therapy with complete lysis of the thrombus in the left internal carotid artery (ICA) and significant reduction of the baseline neurological deficiency. The vessel later on underwent rethrombosis, but the patient's condition remained stable. Acute ICA occlusion, recovery of the major circulation and subsequent rethrombosis were confirmed angiographically. The intraarterial thrombolytic therapy started 12 hours after the apoplexy. The relief of the major artery occlusion in an acute period of cerebral ischemia prevented the development of infarction and promoted adequate collateral circulation due to which subsequent rethrombosis appeared asymptomatic. It is suggested that intraarterial fibrinolytic agents may be effective as well at later postapoplectic periods (after 6 hours and later).
