04/25/2021 - "目的:通过观察慢性萎缩性胃炎(chronic atrophic gastritis,CAG)大鼠胃黏膜细胞凋亡水平及核转录因子-κB(NF-κB)、B淋巴细胞瘤-2(Bcl-2)基因表达变化来探究针灸对CAG的治疗机制。方法:将健康雄性SD大鼠随机分为正常组、模型组、艾灸组、针刺组,每组12只。采用1-甲基-3-硝基-1-亚硝基胍(MNNG)联合多因素法(MNNG+乙醇灌胃+饥饱失常法)造模12周制备CAG大鼠模型。艾灸组予“足三里”“中脘”悬灸15 min,针刺组予 “足三里”“中脘”直刺15 min,均每日1次,连续治疗2周。HE染色法观察各组大鼠胃黏膜形态变化,TUNEL法测定并计算各组大鼠胃黏膜细胞凋亡指数,实时荧光定量PCR法检测各组大鼠胃黏膜组织NF-κB、Bcl-2的基因表达水平。结果:模型组大鼠肉眼下胃黏膜色白淡暗,皱襞低平且有显著出血点,光镜下黏膜固有层腺体萎缩,排列错乱且数量显著减少,炎性细胞浸润;艾灸组和针刺组胃黏膜形态均得到改善。与正常组比较,模型组胃黏膜细胞凋亡指数显著增加(P<0.01);与模型组比较,艾灸组与针刺组胃黏膜细胞凋亡指数均显著降低(P<0.01)。与正常组比较,模型组胃黏膜组织NF-κB、Bcl-2 基因表达显著升高(P<0.01);与模型组比较,艾灸组、针刺组胃黏膜组织NF-κB、Bcl-2 基因表达均显著降低(P<0.05,P<0.01);与艾灸组比较,针刺组胃黏膜组织Bcl-2基因表达明显降低(P<0.01)。结论:传统针灸疗法可以下调CAG大鼠胃黏膜细胞凋亡指数,平衡胃黏膜细胞过度凋亡,降低NF-κB、Bcl-2基因表达,缓解大鼠胃黏膜炎性反应,保护胃黏膜组织,针灸调控胃黏膜细胞凋亡的机制可能与降低NF-κB、Bcl-2基因表达有关。."
