The Utility of Common Blood Coagulation Examination in Identifying Hemorrhoid Complications.

AbstractBACKGROUND:
This observational study was designed to evaluate prothrombin time and activity (PT, PT%), prothrombin time international normalized ratio (INR), activated partial thromboplastin time (APTT), coagulation enzyme time (TT), fibrinogen content (FIB), D-dimer (D-dimer), and platelet count (Plt) in patients with hemorrhoids (HE, n = 207), hemorrhoids with bleeding (HB, n = 59), and incarcerated hemorrhoids (IH, n = 10) and the utility of these parameters in the differential diagnosis of hemorrhoids with thrombosis (HT, n = 30).
METHODS:
Patients were grouped according to the relevant clinical diagnosis and classification standards. Blood coagulation-related tests, platelet counts, and relevant laboratory indicators (PT%, PT, APTT, TT, INR, FIB, D-dimer, and Plt) were carried out with samples obtained from patients with hemorrhoids.
RESULTS:
In the ROC curve analysis, D-dimer could better distinguish HE from HB (AUC = 0.587, p = 0.042). D-dimer (AUC = 0.643, p = 0.011), INR (AUC = 0.630, p = 0.022), and TT (AUC = 0.617, p = 0.038) could distinguish HT from HE, although no significant difference was observed between the ROC curves.
CONCLUSIONS:
D-dimer is useful in distinguishing HE from HB. Moreover, D-dimer, INR, and TT are useful in distinguishing HE from HT. Our findings provide a theoretical basis for future research in this area.
